Why is Dr. O'Connor not seeing patients in Evergreen right now?

Dr. O'Connor's office was devastated by a fire on March 28, 2025. Dr. O'Connor was injured during the fire and has spent the time since the fire recovering from his injuries. His office and all its contents appear to be a total loss.

I was a patient of Dr.O'Connor-where can I go to get treatment?

Dr. Allen Ward in Andalusia has graciously agreed to see Dr. O'Connor's patients during this time. His office can be reached at 334-222-6477. 

May I go to another provider of my choosing?

Yes

Will the new dentist take my insurance?

Please be aware if you are treated in another dental office, you will be subject to their fee schedules. You should check with your insurance company to verify benefits and coverages prior to your appointment.

I am a Medicaid patient- where should I go?

​If you are a Medicaid patient, you should call 1-800-362-1504 to locate a Medicaid provider.

Can I get my Dental Records?

At this time, it appears that all patient records (including dental records, insurance data, etc) were lost in the fire at 104 Hillcrest Dr on March 28, 2025. It also appears that the on-site digital backup of these records was destroyed in the fire.

One way to help rebuild a record is to contact your dental insurance provider and ask if they can provide copies of any claims made on your behalf by Dr. O'Connor.

What happens if you find my records or any information about me when the site is being cleaned up?

Although we believe the chance of finding any records or patient information intact due to the extent of the fire is slim, if anything is found, please rest assured that the documents will be held on to and we will make our best attempt to contact you to determine if you would like them. Please be aware that this process may take some time.
